Biofourmis Care@Home™ is a software solution designed to enable remote care by leveraging software and data science to aid clinical care teams in monitoring and managing adult patients in the comfort and convenience of the home. Care@Home supports several clinical conditions (e.g., heart failure, hypertension, diabetes) and spans the entire care continuum of acute, post-acute, and chronic care.

The Biofourmis Care@Home™ solution also provides a mobile application for healthcare providers to remotely monitor and coordinate care for patients which includes:
